This very petite, 34 year old woman underwent massive weight loss following a gastric sleeve in 2015 going from 180 lbs to 109 lbs. In 2021, she underwent a breast augmentation with a 370 CC high profile gel device placed in the subfascial plane by another Charlotte-based plastic surgeon. Not surprisingly, her implants bottomed out due to the lack of tissue integrity and the size of the implant. 8 months ago, she came to me for removal and replacement of her implants with conversion to a Mentor 200 cc moderate profile plus saline mentor device and a vertical mastopexy. The implants were removed from the subfascial plane and placed in a submuscular, dual plane in part due to correct laterael implant malposition. When lying down, the implants travelled into her armpits. What’s this correction, she now looks much younger and are more age-appropriate and fit her 4’11 frame. Another point to make is it the mastopexy involved resections of breast tissue below the nipple (aka vertical reduction) and creation of a sutured sling to better hold implants in place. A skin only mastopexy does not hold especially for massive wet last patients. This approach also forgoes the expanse many surgeons place on the patients when using biologic mesh to hold oversized implants in place.
